Parents Need To Provide The Following Prior To Enrollment

- A completed application form including general information, emergency and other permissions, phone numbers, the policy, agreement and Guidance & Discipline pages.
- A medical form completed by the child’s physician indicating immunization records and date of last exam, which must be updated every 2 years.
- Any applicable information regarding the child’s home situation or early life which may have a bearing on his/her behavior at school.
- A box/bag containing a complete change of clothes to be kept at school for use as needed.
- A statement of drop off and pick up times, which are not to exceed operating hours.
- A list of persons authorized to pick up the child with each person’s complete contact information and agreement to show proper photo identification when arriving at school.
